Jack passed away at Santiam Memorial Hospital in Stayton, Or. He leaves his wife Helen, daughter Vicki and son-in-law Stephan Weaver. Graveside and inurnment at 1:00pm Wed. May 25th at Fern Hill Cemetery in Menlo. A gathering will follow at Raymond Elks Lodge.
